HIV is a virus that attacks the immune system — specifically CD4 T-cells — and without treatment progresses to AIDS. With modern antiretroviral therapy (ART), people with HIV can live long, healthy lives with an undetectable viral load, meaning they cannot transmit the virus to partners. Treatment has advanced from multi-pill regimens to single-tablet daily pills and, now, long-acting monthly or twice-yearly injections.

1. Start treatment as soon as possible — same day if you can

The current standard is 'treat immediately' — ART is recommended on the day of diagnosis regardless of CD4 count. Early treatment preserves immune function, prevents transmission, and reduces long-term complications. The INSIGHT START trial proved that waiting was worse. Your HIV doctor will prescribe your first regimen the day you're diagnosed or very shortly after. Don't delay.

2. Do NOT skip treatment over cost concerns — financial assistance covers almost everyone

HIV medications are expensive at list price, but no one in the US should be paying list price. If uninsured: apply for ADAP (AIDS Drug Assistance Program) immediately through your state health department. If commercially insured: manufacturer co-pay programs (Gilead Advancing Access, ViiV Compass) bring costs to $0/month. Contact an AIDS Service Organization (ASO) in your area — they navigate this for free.

3. Find an HIV specialist (infectious disease physician)

Primary care can manage stable HIV, but your initial workup, resistance testing, and regimen selection should involve an HIV specialist or infectious disease physician. They'll order a baseline resistance test (HIV genotype) to ensure your first regimen is effective. HIV clinics at academic centers, community health centers (FQHCs), and Ryan White-funded clinics provide comprehensive, often sliding-scale care.

4. Know your key numbers: CD4 count and viral load

Two lab values define HIV management: CD4 count (your immune strength — normal is 500–1500 cells/mm³; treatment goal is to restore and maintain >500) and viral load (copies of HIV RNA per mL of blood — treatment goal is 'undetectable,' typically <20–200 copies/mL). You'll check these every 3 months initially, then every 6 months once stable. Undetectable viral load means ART is working and you cannot transmit HIV to partners (U=U).
